We know you care about your patient’s recovery and want them to receive the best treatment necessary to improve their condition as quickly as possible and so do we! We also know that the last thing your staff needs is more work, so we’ve created a simplistic submission process for you.
You can submit your patients to us and their card can be activated and ready for use within the same day. With their AccidentMeds card, they can purchase any generic, accident-related medications that you prescribe at any local retail pharmacy (CVS, Walgreens, Walmart, etc) at no cost to them until their case settles.
To enroll your patient in AccidentMeds, simply fill out the pdf enrollment form and have the patient sign the lien and waiver below and email these documents to us at firstname.lastname@example.org… You are done!
Once your patient’s AccidentMeds card is active, we will contact your patient and mail them a copy of their card. We will also contact the patient’s attorney to get an attorney signed lien/LOP.
